Sunlight Care provides home health care services for families in Southern New Jersey. We are a family owned agency that is committed to providing the highest level of care to our patients. [Live-In Care]
Live-in home care has become the preferred long term care solution for most seniors who need more than just a couple hours of support. They can remain comfortably in their own homes, maintaining a high level of independence and retain their dignity and quality of life. Its simpler and safer then adult day care solutions since there is no need for transport every morning and evening. It’s cheaper than assisted living facilities and live-in aides provide for the maximum quality care since it is one-on-one care.
Home Healthcare
All home health care services are performed by a Certified Home Health Aides (CHHA) that have been thoroughly screened by Sunlight Care LLC.
Our CHHA will come to your home and provide hands-on assistance with Activities of Daily Living (ADLs), including, but not limited to, the following:
• Dressing and grooming
• Bathing: baths, showers, bed baths
• Rest room assistance
• Transition from bed to chair
• Nail care
• Walking & exercise assistance
• Taking vital signs
• Medication reminders
• Transport to doctors' appointments, shopping
• Light housekeeping, meal preparation
Patient Advocacy:
Effective case management and intervention creates better communication among the patient, family and health care providers. Can you or your loved one answer these questions:
• Why am I taking this medication?
• What should we ask the doctor or pharmacist?
• What are my health care options?
• Who should I contact first?
• What does my insurance cover?
• How can we afford this?
• What are my instructions?
• Do I need a second opinion?
Time is of the essence when the diagnosis is serious and all the confusion usually results in delays. These services are recommended for families who are not able to be present during critical times or for clients and families who are confused about their medical care. These services are performed by a Registered Nurse. Services include but are not limited to, the following:
• Accompany client to appointments
• Explain medical information or treatments
• Update client’s medical history
• Coordinate care among the healthcare team
• Visit in the hospital and advocate for the clients
• Attend care meetings in the rehab.
